Comprehensive Pre-Visit Review
Prior to the initial consultation, an electronic health assessment is provided for your completion. This data, along with a review of medical history and existing diagnostic results, forms the basis for the clinical strategy. This allows the appointment time to be focused entirely on analysis and clinical recommendations rather than data collection.
The Consultation
Appointments are conducted via a secure, web-based telehealth platform. These sessions provide a personalized assessment of cardiovascular and metabolic health, followed by a detailed review of risk factors and actionable recommendations. Each visit concludes with a clear plan regarding lifestyle modifications, medication considerations, and necessary diagnostic follow-up.
Ongoing Support & Care Coordination
Access to a secure messaging portal is provided for non-urgent questions, health updates, and the submission of new test results (duration of access is determined by the selected package). To ensure a cohesive health strategy, clinical findings and recommendations can be coordinated with primary care physicians or other members of the healthcare team upon request.

Kevin Forey MD provides specialized concierge care focused on the prevention and management of metabolic and cardiovascular conditions, including obesity, hypertension, dyslipidemia, insulin resistance, and fatty liver disease.
Primary Care Requirements
This practice does not provide general primary care, urgent care, or emergency services. Because the practice is conducted entirely via telehealth and does not maintain a traditional physical office or daily walk-in hours, it is not equipped to manage acute illnesses, infections, or injuries.
Coordination of Care
All patients are required to maintain a relationship with a local primary care physician or healthcare provider for routine screenings, physical exams, and acute medical needs. Clinical findings and metabolic strategies from this practice can be shared with your local healthcare team to ensure a cohesive approach to your long-term health.
Yes. Prescribing medication, ordering lab tests, and diagnostic imaging, is determined by the level of service selected.
- Comprehensive Assessment:
This is a diagnostic consultation only. No new prescriptions, refills, or medication management are provided. - Treatment Launch Package:
Includes treatment initiation and medication management for a maximum of 120 days. Within this timeframe, clinical oversight for these medications must be transitioned to a local healthcare professional or continued through an Annual Membership. - Annual Membership:
Provides year-round clinical oversight, including medication management and refills for metabolic and cardiovascular conditions.
Scope of Practice
Prescriptive care focuses on metabolic and cardiovascular health, including cholesterol-lowering medications, blood pressure management, and weight loss therapies.
This practice does not manage medications for acute illnesses, infections, or non-metabolic conditions.
Controlled Substances
No controlled substances (such as opioid pain medications or stimulants) are prescribed via this telehealth platform.

This practice operates on a self-pay basis and does not accept or contract with healthcare insurance providers. All services are cash-based, with payment via credit or debit card required at the time of booking to confirm the appointment.
Insurance Reimbursement (Superbills)
While direct insurance billing is not provided, an itemized superbill can be issued upon request. Patients may submit this documentation to their insurance carrier to seek potential out-of-network reimbursement. It is recommended to contact your insurance provider directly to verify your out-of-network member benefits.
Third-Party Expenses
Diagnostic orders (such as laboratory blood work and imaging) and prescription medications are filled through external facilities. These specific expenses may be covered by your health insurance plan, depending on your individual coverage. This practice recommends verifying coverage for third-party diagnostic services with your insurance carrier prior to fulfillment.
At this time, all appointments are web-based telehealth video visit. Appointments can be completed on your computer, tablet, or smart phone. Access to internet is required.
