If Kevin Forey MD is not yet accepting patients in your state, you are welcome to join the Standard Waiting List. You will be notified if care becomes available in your state in the future. Interest from waitlisted individuals helps inform future state expansion priorities.
Priority State Expansion
If you would like to begin working with Kevin Forey MD sooner, you may request Priority State Expansion by enrolling in the Annual Longevity Membership.
The Commitment: Enroll in the Annual Longevity Membership ($1,500).
Required Step: In addition to enrolling in the Annual Longevity Membership, you must complete the Priority State Expansion form below before the licensure process can begin.
Process: Once your membership purchase and form submission are received, Kevin will begin the medical licensure process in your state and will keep you updated along the way.
Timeline: State medical boards control their own review and processing times. In many cases, licensure is completed within several weeks, although some states may take longer.
Membership Start and Refund Policy: Your 12-month membership period does not begin until your state license is active and your first appointment is scheduled. If licensure cannot be obtained, your membership fee will be refunded in full.
