Standard Waiting List
If your state is not currently listed, you can join the standard waiting list. You will be notified once Kevin Forey MD begins treating patients within your state. As more patients express interest from a given state, I will begin prioritizing the medical licensing process.
Expedited Waiting List
Alternatively, if you want to become a patient now, this process can be expedited by submitting your inquiry for the Expedited Waiting List. After submitting your information below, Kevin Forey MD will provide a custom quote to begin the medical licensing process in your state. Generally speaking, this will require pre-payment of 2 or more patient appointments, which is determined by the cost of medical licensing in your state. These appointments can be split up among spouses, couples, relatives, and friends. For many states, a medical license can be obtained within 5-10 business days and patient appointments can be scheduled shortly thereafter. For other states, the licensing process may take as long as 3-6 months. Please submit a free, no commitment inquiry and Kevin Forey MD will be in contact with you shortly.
